We are in network with Sutter Select Health, Cigna and United Healthcare insurance.
To verify your insurance coverage please email us your:
- Name
- Member ID number
- Date of birth
- Insurance plans: HMO, EPO, PPO or email us a picture of your insurance card if unsure.
We offer insurance billing as a courtesy service for our patients but require your documentation of insurance coverage or pre-certification for acupuncture treatment prior to your visit. Thank you.
Pre-authorizations or documentation from prescribing doctors can be faxed directly to our office at: 650-225-0809.
